HODS Request Form

The HODS Request Form is used when submitting samples to the Haematopathology and Oncology Diagnostic Service (HODS) at Cambridge University Hospitals NHS Foundation Trust.

It ensures:

  • Accurate patient identification

  • Clear indication of required tests

  • Provision of relevant clinical information

  • Proper sample labelling and submission

This form is also available in Appendix 1 of the HODS Handbook.

Although the laboratory may accommodate receipt of a local request form, where this facilitates a local laboratory pathway, the HODS form is preferred whenever possible and it is essential that equivalent information fields are provided.
